Compensation payments made to inmates have almost doubled in the past year to more than £4m, official figures show. The Home Office has confirmed this included a claim for £2.7m by one prisoner but they would not confirm it stemmed from a failed suicide bid. Government statistics showed most other payments were made out-of-court for claims relating to injuries, assaults and clinical negligence. A Prison Service spokesman said figures varied "considerably" each year.

Michael Powell, 38, died after he was arrested outside his home in Lozells, Birmingham, on 7 September 2003. A sex attacker who killed mother-of-one Naomi Bryant while out of jail on licence should not have been released, a watchdog has said. Anthony Rice had been freed nine months before he stabbed Ms Bryant, 40, in her home in Winchester in August last year. Chief Inspector of Probation Andrew Bridges said Rice was "too dangerous to be released in the first place". There were "substantial deficiencies" in Rice's supervision by probation and other officials in Hampshire, he said.

A man may have been left dying in a street for many hours because of poor communication from emergency services, investigators said. The Independent Police Complaints Commission (IPCC) is investigating the death of John Lockley. The 61-year-old collapsed near his home in Bentilee, Stoke-on-Trent, on Thursday and died on Saturday. A post-mortem examination showed he died of head trauma but there were no suspicious circumstances.

Hayley Richards called Wiltshire Police when she was attacked in her Trowbridge flat by her boyfriend, Portuguese factory worker Hugo Quintas, in 2005. Officers did not arrest him, despite being told where he was. He cut Hayley's throat a week later.

Rachel Cheesewright, 29, died in the crash near Ashford in October when her Ford Fiesta collided with the police car which was answering a 999 call. The Police Ombudsman's office has begun an investigation after a man was shot dead at a checkpoint in County Down. Steven Craig Colwell, 23, from Main Street, Cullybackey, died after he was shot in a stolen car at Church Street, Ballynahinch, at 1130 BST on Sunday. Three men and two women who were in the car have been released on bail and are being questioned by the ombudsman.

He has always said he did not know that Brazilian Jean Charles de Menezes had been killed until the following day. But the Independent Police Complaints Commission has been told by another senior officer that someone in his office believed six hours after the shooting that the wrong man had been shot.
